Personal data owners defined as the person concerned in the Personal Data Protection Law No.6698 (“ KVKK ”) ( hereinafter referred to as the “ Applicant ”) are granted the right to make certain requests regarding the processing of their personal data in Article 11 of the KVKK.
Pursuant to the first paragraph of Article 13 of KVKK; Regarding these rights, applications to our company, which is the data controller, must be submitted to us in writing or by other methods determined by the Personal Data Protection Board (” Board “).
In this context, the applications to be made to our Company in “written” form, by printing this form;
With the application of the Applicant personally, through a notary, by signing by the Applicant with the ” secure electronic signature ” defined in the Electronic Signature Law No. 5070 and sending it to the registered e-mail address of the Company.
Below, detailed information is given about how and through which channels the written applications will be delivered to us.
- In case of personal application, the written application is submitted to the Company’s Office Address: Manas Bulvarı Adalet Mah. 47/B A Blok 3404 34.Kat Bayraklı Izmir by writing ” Information Request within the Scope of the Law on Protection of Personal Data ” on the envelope .
- If an application is made by sending a notification through a notary public, the written application is submitted to the Company’s Office Address; Manas Bulvarı Adalet Mah. 47/B A Blok 3404 34.Kat Bayraklı Izmir by writing ” Request for Information within the Scope of the Law on Protection of Personal Data ” on the envelope .
- If an application is made via Registered Electronic Mail (KEP) signed with a secure electronic signature, the written application will be made to the info@thehealthstoreturkey.com e-mail account by writing ” Personal Data Protection Law Information Request ” in the subject section of the e-mail ..
In addition, after the announcement of other methods to be determined by the Board, our Company will announce how applications will be received through these methods.
Your applications submitted to us will be answered within thirty days from the date your request is received by us, depending on the nature of the request, in accordance with paragraph 2 of Article 13 of the KVKK ; Our responses will be sent to you in written or electronic form in accordance with the relevant provision of Article 13 of the KVKK.
